What is an Allergy Requisition Form?

Allergy testing custom requisition forms are used by physicians when requesting an evaluation of the immune system’s response to potential allergens. Medical professionals perform skin and blood tests indicating which allergens invoke negative reactions in a patient. The custom integrated label form is sent to a reference lab with specimens to be tested. The physician then analyzes results to identify allergies and treatments.
