Prenatal Lab Requisition Form

What is a Prenatal Requisition Form?

“Prenatal” refers to the period before childbirth. Prenatal testing is sometimes necessary to determine the probability of chromosomal disorders in a fetus. Testing can be conducted using specimens from an expecting mother, including saliva or blood. They can also be performed with fetal specimens such as amniotic fluid, extracted DNA or blood. When a physician orders testing, a prenatal requisition form is sent to a reference lab with samples. Upon completion of testing, a provider or genetic counselor analyzes the results to determines diagnosis and treatment recommendations.
